What To Bring To Your Appointment

We understand that doctor’s visits can create stress and anxiety, but we’re here to help you in any way we can. Let us know how we can make your patient experience a good one.

In order to provide you with the best treatment possible, we need access to certain medical information. Prior to your appointment, please confirm that your doctor sends us your records or provides you with copies to bring to your first appointment. Also, please make sure that any referral or pre-authorization requests are completed by your doctor prior to your appointment.

  • Blood work from the past 2 years
  • Urine specimen results
  • Renal ultrasound reports
  • Recent blood pressure readings
  • List of allergies
  • Current medications in their original bottles
  • Current insurance (including cards or copies of cards)
  • Referral or pre-aurhorization paperwork (if applicable)

Office Policies

– Prescriptions and refills: All requests for prescriptions and refills require a minimum 24-hour notice. Prescription needs are accepted during regular office hours. When you call in for refills, please have on hand the name of your medication, its dosage and frequency, in addition to the name and telephone number of your pharmacy. Our doctors will only renew prescriptions that were written or initiated by them. This office will not call in prescriptions for narcotics and/or sleeping pills.

– Medical Questions: During office hours, if you have medical questions that only your nephrologist can answer, a message will be taken and your medical file will be given to the doctor. The doctor or qualified staff member will return your call as quickly as possible.

– Cancellations: In the event you are unable to keep a scheduled appointment, we ask that you kindly notify our office at least 24 hours in advance. This courtesy on your part will make it possible to give your appointment to another patient.

– Emergencies: Our office provides on-call service 24 hours-a-day, 7 days-a-week. All medical emergencies that are life threatening or require immediate attention should be directed to the nearest emergency room. The emergency room personnel will contact the nephrologist covering that facility.
